The Dayton VA Medical Center is seeking a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) to join our clinical team in Middletown, Ohio. This position will assess, plan, implement, and evaluate care based on age-specific components. Also assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
