The Home Based Primary Care/ Geriatrics Extended Care Registered Nurse Case Manager will provide initial and continued assessment of nursing needs of the patient; teach the Veteran and caregiver effective and efficient ways to manage and deliver care in the home and monitor the Veterans' condition and the care provided in the home.
