The Dayton VA Medical Center is looking for a Medical Instrument Technician (Non-Invasive EKG) within Nursing Service under the direct report of the Cardiology Nurse Manager. This is a bargaining unit position. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. English Language Proficiency. Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: (a) Experience: Candidates must: Have one year of creditable experience equivalent to the GS-06 grade level which includes but not limited to: Ability to recognize special patient needs such as disabilities, impaired mobility, and complicating medical conditions in order to prepare and position the patient for the prescribed procedure. Knowledge of the specialized equipment and accessories appropriate to Holter monitoring and ability to perform such tests. Knowledge of universal precautions and sterilization techniques and ability to follow CDC guidelines in cleaning equipment. Ability to monitor the patient for adverse reactions and take appropriate action. Ability to operate the equipment to perform standard and routine highly specialized procedures including exercise stress testing. AND (b) Demonstrated Knowledge, Skills and Abilities. In addition to the requirements above, candidates must demonstrate all the following KSAs: Knowledge of typical patient reactions and signs of distress including the ability to recognize, report and treat potentially lethal arrhythmias. Knowledge of common equipment settings and standardized procedures plus knowledge of common errors and corrective measures. Ability to modify procedures/positions to obtain the correct results with patients with complicating conditions such as amputations, Parkinson's disease, structural defects, and scar tissue. Ability to act as a mentor or preceptor to lower graded technicians. Ability to conduct in-service training on the EKG equipment and related instrumentation. Physical Requirements: The Dayton VA Medical center is a major interdisciplinary teaching facility, serving medical, surgical, inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continues walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. ["Major duties include but not limited to: Performs all non-invasive cardiology examinations including EKG, Holter/Event monitoring, cardiology research and assists in the cardiology clinics, Stress testing including nuclear medicine stress testing, tilt table studies, and any other duties assigned by the supervisor. Records EKG's of diagnostic quality; observing proper lead placement and patient demographics, having the ability to deviate from the standard procedure as the situation calls for a 95% accuracy, is required. Takes 12 lead EKG, rhythm strips, knowledgeable of use of magnet for pacemaker EKG. Perform EKG's and rhythm strips in pacemaker clinic. Advises physician of results of exam and confers with cardiologist for difficult exams and abnormal findings. Connects patient to the EKG/Blood Pressure and operates the treadmill. Performs graded exercise testing of many different protocols: (Bruce, modified Bruce, Thallium, Persantine, Dolbutamine and Research). Train physicians and cardiology staff on the operation of the stress machine, procedure, and analysis of the data. Recognizes rhythm abnormalities and ST segment changes before, during and after the exercise period. Accurately applies Ziopatch for continuous monitoring/recording ECG data for analysis of cardiac rhythm disturbances, registers patient with company, transcribes data in patient record, contacts physician immediately with event alerts. Routinely verifies transmission of EKG, Stress data to Cardiology computer network, Via the MUSE Cardiology system. Performs other duties as assigned.
