The Social Worker is assigned to the Health Care for the Homeless Veterans (HCHV) Program. This position provides Outreach/case management and intervention support to the Health Care for Homeless Veterans (HCHV) Program in a team oriented, recovery-based Program. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
